Job Description
This PRN Nurse Practitioner role involves conducting in-home health risk assessments for adult and geriatric patients, including reviewing medical history, medications, social factors, and health status during visits that can last up to one hour.
Responsibilities include collaborating with physicians, case managers, and caregivers; providing health education; making referrals; and documenting assessments according to protocols.
Work environments include patient homes, skilled facilities, and community settings in Dover, Ohio, with flexible hours including days, evenings, nights, and weekends.
Earn from $1,900 to $9,500 per month based on assessment volume; compensation includes mileage reimbursement and limited voluntary benefits.
Qualifications require a Master’s degree or equivalent experience, current RN and NP licensure with prescriptive authority, certification in a Matrix-approved specialty, and CPR/BLS/ACLS certification. New grads encouraged to apply.
