Seeking a Nurse Practitioner for PRN home risk assessments in Texas, with new grads welcome.
Conduct up to one-hour assessments of adult/geriatric patients in their homes or facilities.
Review medical history, medications, social factors, and health risks to improve care quality and close care gaps.
Collaborate with physicians, case managers, social workers, and family members.
Provide follow-up, health education, and referrals as needed.
Work flexible hours, including days, evenings, nights, and weekends, in community settings in Orange, TX.
Earn $2,000-$10,000/month based on assessments completed.
Qualifications include a Master’s or equivalent experience, current RN and NP licensure, board certification, and valid driver’s license.
Benefits include mileage reimbursement; PRN providers receive limited benefits.
Travel required to meet patients at their locations.
