Job Description
This PRN Nurse Practitioner role involves conducting comprehensive home risk assessments for adult and geriatric patients, including medical history, diagnosis, treatment, health education, and referrals.
Assessments may be performed in homes, skilled facilities, or community settings across Indiana and Pennsylvania.
Work collaboratively with physicians, case managers, social workers, and caregivers, providing follow-up and patient education.
Flexible scheduling includes days, evenings, nights, and weekends, with pay ranging from $2,000 to $10,000 monthly based on completed assessments.
Qualifications include a master’s degree or equivalent experience, current RN and NP licensure, board certification, CPR/BLS/ACLS, and required travel ability.
New grads encouraged; mileage reimbursement included. Benefits are limited to 401(k) and voluntary options.
