Job Description
Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in New York. New grads encouraged to apply and earn a sign-on bonus of $1,500.
Conduct comprehensive adult/geriatric health assessments in patients’ homes, SNFs, and community settings.
Collaborate with physicians, case managers, social workers, and caregivers.
Review medical history, medications, social factors, and identify health risks to improve care and close gaps.
Flexible hours including days, evenings, nights, and weekends, with potential earnings between $2,300 and $11,500 monthly based on assessment volume.
Qualifications include a Master’s degree or equivalent experience, current licensure, board certification, CPR/BLS, and ability to travel.
Benefits include mileage reimbursement and 401(k); PRN providers do not receive full benefits.
Travel and flexible scheduling are required to meet patients where they live.
