Job Description

Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in Greenville, NY, with new grads encouraged to apply and a $1,500 sign-on bonus.

Conduct comprehensive health assessments in patients’ homes, reviewing medical history, social factors, and risks to improve care and close health gaps.

Responsibilities include adult/geriatric assessments, collaborating with healthcare teams, providing patient education, and documenting findings.

Pay ranges from $2,300 to $11,500 monthly based on assessments completed.

Flexible hours including days, evenings, nights, and weekends. Benefits include 401(k) and mileage reimbursement; PRN providers are non-benefited.

Qualifications: Master’s degree or equivalent experience, current licensure, certification, prescriptive authority, and travel ability. Strong communication and computer skills required.

Travel to patients’ homes is necessary; valid driver’s license and auto insurance required.
