Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in New York. New grads are welcome, with a $1,500 sign-on bonus.

Conduct comprehensive adult/geriatric health assessments during home or facility visits, reviewing medical history, social factors, and risks.

Collaborate with healthcare teams and PCPs to provide patient education and follow-up.

Work flexible hours—including days, evenings, weekends—in various community settings.

Earn $2,300 to $11,500/month based on assessments completed, with mileage reimbursement.

Qualifications include a Master’s Degree or equivalent experience, current licensure, certification, and ability to travel.

Must have strong communication skills, computer proficiency, and a positive, team-oriented attitude.

Travel is required to meet patients at their homes or community sites.
