Nurse Practitioner or Physician Assistant – PRN

Job Description

Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments, with new grads encouraged to apply and a $1,500 sign-on bonus.

This role involves conducting comprehensive assessments during visits up to an hour, reviewing health, medical history, medications, and social factors to improve care and identify undiagnosed conditions.

Responsibilities include performing adult/geriatric assessments, collaborating with healthcare teams, providing patient education, and documenting according to protocols.

Work locations include home, skilled facilities, and community settings in Ogdensburg, NY, with flexible hours including days, evenings, nights, and weekends.

Compensation ranges from $2,300 to $11,500/month based on assessment volume. Benefits include mileage reimbursement, 401(k), and limited voluntary benefits.

Qualifications: Master’s degree or equivalent experience, valid RN/NP/PA licensure, board certification, CPR/BLS, travel ability, and computer skills. Travel and valid driver’s license required. Equal opportunity employer.
