Job Description

Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments, with new grads welcome and a $1,500 sign-on bonus.

  • Conduct comprehensive adult/geriatric assessments in patients’ homes, skilled facilities, and community settings.

  • Review medical history, medication adherence, social factors, and health risks to improve care quality and close care gaps.

  • Collaborate with physicians, case managers, and caregivers, providing follow-up and health education.

  • Flexible hours including days, evenings, nights, and weekends. Potential earnings range from $2,300 to $11,500/month based on assessments completed.

  • Must have current licensure, board certification, CPR/BLS, and ability to travel within NY. New grads encouraged to apply. Mileage reimbursement provided.
