Seeking Nurse Practitioners or Physician Assistants for PRN home risk assessments in Virginia. New grads are encouraged to apply and receive a $1,500 sign-on bonus.

Conduct comprehensive assessments in patients’ homes, reviewing health, history, medications, and social factors to improve care quality and address care gaps.

Responsibilities include medical evaluations, patient education, referrals, and documentation, collaborating with healthcare teams, and working in various community settings.

Compensation ranges from $2,200 to $11,000 monthly, based on assessment volume, with flexible hours including days, evenings, nights, and weekends.

Qualifications: Master’s degree or equivalent experience, current licensure, certification in BLS/CPR, valid driver’s license, and ability to travel.

Benefits include mileage reimbursement; PRN providers receive limited benefits.

Travel required to meet patients at home or community sites.
