Earn from $2,200 to $11,000 monthly based on assessments completed.
Conduct in-home evaluations lasting up to one hour, reviewing health, history, medications, and social factors.
Work collaboratively with healthcare teams, providing patient education and follow-up.
Flexible schedule including days, evenings, nights, and weekends.
Positions are in-home, SNF, and community settings in Hampton, VA.
Qualifications include a Master’s degree or equivalent experience, current licensure, board certification, and ability to travel.
