Job Description
Seeking a full-time Nurse Practitioner or Physician Assistant for home risk assessments across New York and bordering states.
Conduct comprehensive assessments including medical history, diagnosis, treatment, and education in patient’s home or community settings.
Collaborate with physicians, social workers, and caregivers to improve patient care and close care gaps.
Travel up to 80-90% within the state, including overnight stays, with flexible scheduling Monday–Friday, 8am–5:30pm.
Qualifications include a Master’s or equivalent experience, current licensure, board certification, and 3+ years of clinical experience.
