Job Description
Seeking a full-time Nurse Practitioner for traveling home risk assessments across Pennsylvania and bordering states.
Conduct comprehensive adult/geriatric assessments, including medical history, diagnosis, treatment, and health education.
Collaborate with physicians, social workers, caregivers, and other medical staff.
Provide services in home visits and skilled facilities, with visits lasting up to an hour to gather detailed health and social insights.
Work Monday-Friday, 8:00 am – 5:30 pm, with flexibility; travel up to 80-90%, including overnight stays.
Salary ranges from $111,700 to $142,800 annually, plus benefits like medical, dental, vision, 401K, PTO, holidays, life insurance, and mileage reimbursement.
Qualifications include a Master’s degree or equivalent, current RN and NP licensure, board certification, 3+ years experience, and ability to travel extensively.
