Full-time Nurse Practitioner position in Georgia, welcoming new grads to apply.
Conduct comprehensive home risk assessments, including health history, diagnosis, treatment, and patient education.
Collaborate with physicians, social workers, and caregivers, providing follow-up care and referrals.
Work in various settings like homes and skilled facilities in Columbus, GA, with hours Monday–Friday, 8am–5:30pm.
Salary ranges from $100,100 to $123,800 annually, plus incentives.
