Nurse Manager, Case Management – Relocation Offered!

Job Description

Responsible for leading the implementation and operational management of the Special Needs case management program.
Acts as a liaison between clinical staff and IT to optimize workflows and system use.
Oversees a team of RNs and Social Workers, providing coaching, training, and performance management.
Develops department goals, ensures compliance with regulations, and maintains relationships with external agencies.
Facilitates access to healthcare services, advocates for enrollees, and addresses care coordination issues.
Leads staff meetings, promotes quality improvement, and supports community outreach.
Requires a Bachelor’s degree, RN license in DC, CCM certification, 3-4 years of case management and clinical experience, with supervisory experience preferred.
Skills include strong communication, proficiency in Microsoft Office, and knowledge of healthcare regulations and population health trends.
Salary range: $100,588 – $190,340.
