Job Description

This role involves supporting the Memory Support Program by ensuring resident-centered care aligned with the program philosophy.

  • Collaborate with clinical teams to coordinate resident moves and updates.
  • Complete and share resident profiles and support educational groups for residents and families.
  • Coordinate care changes with wellness and assisted living teams, and prepare care plans and transfer notices.
  • Maintain readiness for licensing and complete required training.
  • Requires 1-2 years of dementia care experience and at least 1 year of supervisory experience or equivalent training.
