Job Description
This role involves supporting the Memory Support Program by ensuring resident-centered care aligned with the program philosophy.
Collaborate with clinical teams to coordinate resident moves and updates.
Complete and share resident profiles and support educational groups for residents and families.
Coordinate care changes with wellness and assisted living teams, and prepare care plans and transfer notices.
Maintain readiness for licensing and complete required training.
Requires 1-2 years of dementia care experience and at least 1 year of supervisory experience or equivalent training.
