Job Description

Join a certified Great Place to Work as an in-person floater supporting MDS/NAC needs across CT facilities with local travel and no overnights.

  • Complete and ensure accuracy of MDS assessments to support personalized care plans and clinical reimbursements.

  • Assess nursing needs of prospective residents, determine PDPM and expense, monitor CMI scores, and ensure compliance with regulations.

  • Collaborate with interdisciplinary teams, provide staff education, and contribute to revenue integrity.

  • Qualifications include a valid RN license, experience in long-term care or MDS, knowledge of regulations, and strong organizational and communication skills.
