Primary Care Plus is seeking qualified Internal Medicine and Family Practice physicians to join our expanding practice in Louisiana. Our clinics are part of a clinically integrated network focused on delivering high-quality, coordinated care to Medicare Advantage and ACO members.

We utilize advanced electronic medical records and data analysis to identify high-risk patients, ensuring improved outcomes. Physicians are evaluated based on patient outcomes rather than productivity, with manageable patient panels and longer appointment times.

The Primary Care Physician (PCP) will provide direct patient care, participate in clinical rounds, and collaborate with a multidisciplinary team. Adherence to departmental goals and regulatory compliance is essential.

Primary Responsibilities:

  • Developing care plans and providing patient education
  • Ordering and interpreting lab and diagnostic studies
  • Prescribing medications and treatments
  • Referring patients to specialists as needed
  • Documenting care through progress notes
  • Participating in patient care rounds
  • Communicating management strategies with the care team
  • Collaborating with the multidisciplinary team
  • Initiating life-saving procedures when necessary

Required Qualifications:

  • M.D., D.O., or M.B.B.S.
  • Board certification or eligibility in Family or Internal Medicine
  • Active medical license in Louisiana
  • Valid DEA license
