Registered Nurse (RN) Case Manager – Home Care: Experience independence and flexibility while visiting patients in their homes.
Hours: Full-time | Days | Monday – Friday | Holiday rotation (approx. two holidays/year) | Weekend rotation may apply based on staffing.
Territory: Fieldwork in the Northeast Twin Cities Metro area, with the office located in Roseville, MN.
Pay and Benefits:
- Annual Salary: $84,905 – $101,233

Requirements:
- Graduate of an accredited nursing program with a current MN RN license.
- Reliable transportation and valid driver’s license.
- CPR Certification.
- Two years of nursing experience; one year in case management preferred.
- Preferred: Community health and Medical-Surgical experience.
Key Responsibilities:
- Conduct assessments and procedures based on nursing principles.
- Build relationships with discharge planners and social workers.
- Evaluate care quality and review clinical notes.
- Participate in interdisciplinary patient assessments.
