We’re seeking innovative, entrepreneurial Primary Care Physicians to provide high-quality, outcome-driven care for seniors.
Accountable for managing a panel of up to 450 patients, focusing on clinical outcomes, cost-effectiveness, and building trusting relationships.
Act as a health coach by helping patients set and achieve health goals through education and behavior change.
Simplify complex health information to promote actionable steps for patients and care teams.
Provide comprehensive care for acute and chronic conditions, collaborating with specialists and care teams, especially for hospitalized or homebound patients.
Lead care teams, participate in quality and risk management, and promote a positive center culture.
Maintain high standards of professional competency, evidence-based medicine, and physician leadership.
Requirements include MD/DO in relevant specialties, active medical license, board eligibility/certification, DEA number, and BLS certification.
