Job Description

The position is for a Master Social Worker to join a home health care team, responsible for evaluating, coordinating, and managing social work services for patients and families.

Schedule: Full-Time, Monday – Friday, serving Middle Tennessee Counties.

Key responsibilities include assisting clients with understanding social and emotional factors affecting their health, developing and implementing care plans, linking clients to community resources, reviewing progress, and ensuring proper documentation and communication with care teams. They will also educate clients and caregivers, coordinate agency services, and contribute to policy development.

Qualifications include a current state license, a Master’s degree in Social Work, at least one year of medical social work experience, and valid CPR.
