Position Overview:
The Registered Nurse provides and documents skilled nursing care per care plans and physicians’ orders, maintaining confidentiality and professional boundaries.
Territory: Luzerne County
Essential Job Functions:
Develops care plans with medical interventions and goals based on assessments and physician input.
Educates clients and families tailored to their needs.
Trains primary caregivers to optimize client functioning.
Coordinates care through documentation and communication with physicians and caregivers.
Provides in-home care including assessments, wound care, infusions, catheter care, and PICC line dressing changes.

Requirements:
Active RN license in the state of application
Valid CPR certification
Preferred:
Experience with Medicare Skilled Nursing
Basic understanding of Oasis
One year RN experience in healthcare setting
