Seeking a Registered Nurse with OASIS experience and HCHB preferred to provide skilled nursing care in patients’ homes.

Develop and implement care plans based on physician orders, assessments, and client input, including measurable goals.

Educate clients and families, and train primary caregivers for optimal patient functioning.

Coordinate care through documentation and communication with physicians and caregivers.

Provide services such as wound care, infusions, catheter care, and PICC line dressing changes, along with patient education and assessment.

Preferred: Medicare Skilled Nursing experience, basic Oasis knowledge, 1-year RN experience.
