Job Description

The Registered Nurse – Home Health provides skilled nursing care in patients’ homes, following care plans and physician orders while maintaining confidentiality and professional boundaries.

  • Full-time position with visits Monday – Friday, covering Columbia, Hamilton, and Suwannee areas.

  • Develops care plans based on assessments, physician input, and client needs, including medical interventions and measurable goals.

  • Educates clients and families, trains primary caregivers, and coordinates ongoing care through documentation and communication.

  • Provides various home health services such as assessments, wound care, infusions, and catheter care.

  • Offers team collaboration, career growth opportunities, and flexible patient interaction to improve outcomes.
