Job Description

The Registered Nurse – Home Health provides skilled nursing care in patients’ homes, following care plans and physician orders while maintaining confidentiality and professionalism.

  • Develop and implement individualized care plans with measurable goals based on assessments and physician input.
  • Educate patients and families tailored to their needs.
  • Orient and train primary caregivers to optimize patient care.
  • Coordinate ongoing care through documentation and communication with healthcare providers.
  • Deliver home care services such as patient education, wound care, infusions, catheter care, and dressing changes.

  • Active RN license and valid CPR required.
  • Preferred: Medicare skilled nursing experience, basic Oasis knowledge, 1-year RN experience.
