This full-time Day shift Physical Therapist Care Manager provides home health services to patients in specific Orlando areas, coordinating personalized care plans based on assessments and patient needs.
Evaluates patient function and develops individualized, measurable goals involving patients, families, and physicians.
Performs regular reassessments, monitors progress, and documents findings to ensure optimal outcomes.
Coordinates care, manages caseloads, and prioritizes visits to support efficiency and quality care delivery.
Requires a Master’s Degree, 1 year clinical experience, driver’s license, and BLS certification; preferred qualifications include a Doctorate and home health care certifications.
