The Home Health Physical Therapist Care Manager provides comprehensive in-home therapy services, assessing patient needs, developing personalized care plans, and coordinating interdisciplinary care.
Responsibilities include evaluating function, establishing realistic goals, and reassessing progress every 30 days.
The role involves educating patients, families, and caregivers to promote independence and monitor treatment effectiveness.
Qualifications include a Bachelor’s Degree, active PT license, BLS certification, and a valid driver’s license. Preferred qualifications are a Master’s or Doctorate Degree and certification as an Oasis Specialist-Clinical (COSC).
This full-time, day-shift position requires independent management of a caseload, ensuring quality care, documentation, and adherence to standards.
