Seeking a full-time or per diem Home Health Physical Therapist in Towson, Maryland, to provide rehabilitative care and skilled nursing at clients’ homes.
Perform home visits, diagnostic tests, and develop individualized care plans including manual therapy, gait training, and balance exercises.
Assess and revise care plans, educate clients and families, and document interventions using electronic records.
Qualifications include a Maryland PT license, graduation from an accredited program, and home health experience for per diem roles.
Strong interpersonal, organizational, and computer skills are essential.
