Seeking a Home Health Physical Therapist in Glendale, CA for an ongoing, staff position starting ASAP.
Job involves planning and conducting treatment programs to improve patients’ physical, social, and mental functions, serving as a co-leader in interdisciplinary teams.
Requirements include a Bachelor’s degree (Master’s preferred), state licensure, CPR/BLS certification, a valid driverâs license, and PT credentials.
Essential functions include performing evaluations, developing individualized treatment plans, delivering therapy interventions, ensuring proper documentation, supervising assistants, and other duties.
