Job Description
This ongoing full-time Home Health Physical Therapist position in Mequon, WI involves conducting patient evaluations, developing and updating care plans, and providing interventions to improve patient outcomes.
Responsibilities include managing caseloads, coordinating with physicians, educating patients and caregivers, and participating in quality improvement activities.
Supervises Physical Therapist Assistants and performs joint visits with Home Health Aides.
Requires licensure in Wisconsin or Illinois, BLS certification, and a Doctorate in Physical Therapy with at least 1 year of clinical experience.
Must have strong clinical skills, good communication, and physical stamina for patient-related tasks, including lifting and mobility.
