POSITION SUMMARY

A home health physical therapist (PT) evaluates and treats patients recovering from injury, disease, or surgery in the patient’s home environment. They establish treatment plans and treat patients using a variety of methods, including exercises, education, mobility training, modalities, and equipment education. The home health physical therapist manages a wide variety of diagnoses and disabilities in a home setting in accordance with a physician’s plan of treatment, considering individual patients special or age-related needs. Work schedules are determined to meet patient care needs and work may be required on weekends and holidays. This job description applies to both scheduled and Per Diem PT roles. The minimum scheduled role is 0.5 FTE and the minimum work expectation for a Per Diem role is 1 shift/month. Per Diems are classified as an hourly/non-exempt employee.

ESSENTIAL JOB KNOWLEDGE

Evaluates and re-evaluates patients using knowledge of the principles and methods of patient assessment and on-going reassessment to develop and modify effective treatment plans.
With patient and family input, establishes goals that reflect anticipated outcomes.
In adherence to the profession’s Code of Ethics, uses ethical reasoning to make decisions based on objective principles and values.
Is accountable and takes responsibility to identify and meet the needs of the patient. Seeks alternate strategies and other resources to make sure patient needs are met.
Provides and progresses therapeutic interventions to patients that are safe, timely, effective, efficient, patient-centered, and equitable, in compliance with organizational policies and procedures.
Recognizes cultural factors that may impact treatment and plan of care and accesses appropriate resources to meet cultural needs and maximize patient outcomes.
Facilitates patient/client responsibility for self-care.
Utilizes resources and seeks appropriate assistance to incorporate evidence-based practice into clinical decision making.
Demonstrates a commitment to continued learning by attending continuing education, reviewing the literature, and seeking other resources.
As opportunities exist, actively participates in student clinical education.
Advocates for the profession internally and externally.
Successfully completes initial and ongoing organizational and department education, training, and competency requirements.
Infection Control: Understands, demonstrates, and follows organizational infection control guidelines (CDIC).
Quality: Focuses on improving processes and champions efficiency in daily work activities.
Safety: Promotes a culture of safety by taking accountability for individual actions, including but not limited to, promptly reporting errors or near misses via the organization’s occurrence reporting system and participating in developing improved safety processes, as appropriate. (ADM015, ADM016, OPS044, Mission Statement).
Performs other duties as assigned.

JOB REQUIREMENTS

Licenses:

State of Wisconsin Driver’s License.
State of Wisconsin Professional licensure for Physical Therapist.

Education:

Bachelor’s degree in physical therapy required.
Graduate degree in physical therapy preferred.

Experience:

A minimum of 1 year of recent experience in physical therapy is required.
Experience and strong knowledge base in home health therapy preferred.

Miscellaneous:

Organization, time management, flexibility, and a positive attitude are crucial to successful performance.
CPR/BLS Certification required.
