Job Description

WVU Medicine is seeking a qualified candidate for a position focused on coordinating care for patients across the continuum. Key responsibilities include performing psychosocial assessments, crisis intervention, resource management, and discharge planning while collaborating with a multidisciplinary team. Candidates must hold a Master’s in Social Work and relevant West Virginia certification (LGSW/LCSW/LICSW). Preferred experience is 1-3 years. The role requires excellent communication skills, the ability to work with diverse populations, and independent judgment regarding patient needs. Occasional on-call and weekend shifts are required.
