Case Manager Workflow Lead

Job Description

The Case Management Workflow Lead is responsible for developing and implementing patient care plans in collaboration with Discharge Planners (DCP), physicians, nurses, and the multidisciplinary healthcare team. This role involves coordinating services, facilitating communication among patients, families, and healthcare team members, and ensuring continuity of care in acute and post-acute settings. Key responsibilities include assessing patients’ needs, initiating referrals, resolving care-related issues, and managing documentation for reimbursement. Candidates should have a Bachelor’s in Nursing or a Master’s in Social Work, four years of acute care case management experience, and relevant certifications.
