Home Care Registered Nurse – PRN

Job Description

Purpose: Provide health promotion, prevention activities, and home health nursing services to individuals, families, and communities.

Responsibilities:

• Assess populations and gather data for health assessments.
• Plan and provide nursing services, including education, case management, and advocacy.
• Maintain accurate documentation and contribute to statistical data collection.
• Develop comprehensive care plans with measurable goals.
• Participate in community outreach and represent the agency.
• Support agency mission and engage in professional development.

Qualifications:

• Valid Iowa RN license and accredited nursing degree.
• One year of clinical experience required.
• Strong communication, critical thinking, and assessment skills.
• Ability to work collaboratively and with diverse populations.
• Current CPR certification and mandatory reporter training.

Working Conditions: Medium physical demands, flexible schedule, and travel required.
