Overview: The role involves engaging with patients and healthcare professionals to optimize therapeutic outcomes, including dispensing medications, monitoring drug therapies, and providing consultations and education.
Responsibilities:
Gather and reconcile medication histories.
Conduct comprehensive medication reviews and make evidence-based recommendations.
Identify adverse drug reactions and adherence issues.
Document clinical activities accurately.
Consult with medical professionals and patients on medication therapy.
Participate in quality improvement and medication use reviews.
Deliver presentations on best practices.
Supervise pharmacy technicians and manage pharmacy systems.
Qualifications:
ACPE-accredited pharmacy degree; Pharm D preferred.
Independent work capability and strong communication skills.
Current NH pharmacist license required.
