Faith Home Health & Hospice is seeking a PRN Physical Therapist (PT) to join their team in West Des Moines, IA. The ideal candidate will be a licensed Physical Therapist with a minimum of 2 years of clinical experience, preferably in home healthcare. Responsibilities include evaluating patients, establishing treatment plans, coordinating care with physicians, and providing patient and family education. Candidates must have a valid Iowa PT license, CPR certification, and reliable transportation. The position offers a supportive work environment, mileage reimbursement, and a pay range of $70.00 – $110.00 per hour.
