Job Description

Lake Woods Nursing & Rehab Center is seeking a full-time RN Clinical Care Coordinator to oversee and coordinate nursing care in compliance with facility policies and regulations. Key responsibilities include:

• Conducting interviews and collecting data for Minimum Data Set (MDS) assessments.
• Making referrals to healthcare professionals and participating in interdisciplinary meetings.
• Supervising resident care and assisting physicians during rounds.
• Engaging in the Quality Assurance process.
