The Physical Therapist (PT) provides intermittent physical therapy to patients in various settings, following physician orders and care plans under the supervision of the Regional Director of Therapy, Outpatient.
Responsibilities:
- Assess patient functional abilities and develop treatment plans.
- Document evaluations and treatment plans for physician approval.
- Coordinate with the interdisciplinary team and supervise Physical Therapy Assistants.
- Instruct patients and caregivers on therapy practices.
- Communicate patient progress and changes to the healthcare team.
Requirements:
Bachelor’s degree in Physical Therapy, valid state license, and preferred two years of experience.
Work Environment:
Indoors in various facilities and patient homes, with travel required.
