Warsaw Meadows Care Center in Warsaw, IN, is hiring an experienced MDS Assessment Coordinator (Registered Nurse) for its 80-bed skilled nursing facility. This full-time nurse manager role offers a flexible Monday-Friday schedule (8:00 AM – 4:30 PM) and includes on-call nursing rotations.

Responsibilities include:

Conducting resident assessments per state and facility policies.

Monitoring condition changes and communicating with staff.

Completing MDS sections and care plans.

Maintaining accurate records and transmitting MDS to the state.

Qualifications: Current RN license and at least 1 year of MDS experience in long-term care.
