Job Title: Transitional Care Coordinator – Behavioral Health Network

Overview: Catholic Charities of Maine is seeking a Full-Time Transitional Care Coordinator to join their Behavioral Health Network, which supports adults and families facing mental health and substance use challenges. This role involves providing care coordination services as part of the C.A.R.E.S. Team, assisting individuals with assessments, stabilization, and service transitions.
