You will provide personalized care to seniors and patients with complex needs in their homes or care facilities.

Responsibilities:

• Deliver evidence-based medical care and evaluate patient needs.
• Examine and treat chronic and urgent illnesses.
• Order and interpret diagnostic tests.
• Collaborate with healthcare professionals for comprehensive care.
• Maintain up-to-date patient medical records.

Qualifications:

• Active NP license and National Board Certification.
• Master’s degree in Nursing and five years of nursing experience.
• Strong communication and time management skills.
• Proficiency in EMR systems and medical regulations.
• Valid driver’s license and ability to engage in moderate physical activity.
