Key Responsibilities:

• Provide home health services, including admissions, re-certifications, and discharges.
• Develop and implement the Physician Plan of Care in collaboration with healthcare professionals.
• Document patient care using a tablet within 24-48 hours.
• Ensure compliance with health and safety guidelines and agency standards.

Qualifications:

• Valid nursing license and proven nursing experience.
• Pediatric experience and a vehicle for travel.
• Experience with OASIS assessments and home care is preferred.
