Grand Oaks Nursing & Rehabilitation Center is hiring a Clinical Care Coordinator to enhance resident care. This full-time position offers a $12,000 sign-on bonus and competitive wages ranging from $44.45 to $50.45 per hour, plus additional pay until 09.30.25. Responsibilities include assessing residents’ physical and mental functions, completing minimum data set forms, making referrals, and overseeing clinical coordination. Candidates must be Registered Nurses (RNs) with strong customer service and interpersonal skills. The center values teamwork and aims to provide excellent care in a supportive environment.
