Position Summary: We are seeking an experienced Physical Therapist (PT) who is passionate about providing functional rehabilitation in real-life settings, such as homes, schools, and workplaces. The ideal candidate thrives in an autonomous environment and enjoys collaborating with a dynamic team of therapists.

Key Responsibilities:

• Conduct evaluations and develop individualized treatment plans.
• Regularly assess treatment effectiveness and participate in team meetings.
• Perform discharge evaluations and provide safety recommendations.
• Communicate patient progress with the treatment team and family.

Qualifications:

• Bachelor’s degree in Physical Therapy and current state license.
• Minimum one year of experience preferred.
• Knowledge of rehabilitation techniques for neurological injuries.
• Ability to lift and transfer patients safely.
