Job Responsibilities:

• Communicate daily with office staff to accept new patients.
• Complete necessary paperwork and submit payroll.
• Maintain open communication with patients and home health agencies.
• Collaborate with Physical Therapists to manage patient care.
• Complete daily visit notes and other required documentation.
• Attend training and team meetings as needed.

Job Requirements:

• Current Physical Therapist Licensure in Michigan.
• Degree in Physical Therapy from an accredited institution.
• Ability to travel to patients’ homes or facilities.
• Strong communication skills.
• Home healthcare experience preferred.
