MDS Coordinator (RN/LPN)

Job Description

We are seeking a full-time MDS Coordinator to join our team, offering same-day pay and a sign-on bonus.

Responsibilities:

• Ensure timely and accurate MDS assessments.
• Verify compliance with regulatory requirements.
• Supervise MDS data entry and resolve data issues.
• Prepare reports for the Director of Nursing.
• Participate in audits and maintain regulatory compliance.
• Support quality improvement initiatives.

Qualifications:

• COVID-19 vaccine required (NE only).
• Graduate of an accredited nursing program (RN, BSN, or LPN).
• Active RN license and 3 years of long-term care experience.
• Prior MDS/RAI experience and strong clinical assessment skills.
• Knowledge of Medicare/Medicaid regulations.
