We are seeking a compassionate MDS Coordinator to join our Skilled Nursing team in Rio Rancho. This role involves creating care plans based on comprehensive resident assessments through interviews, physical evaluations, and interdisciplinary team input.

About You:

  • Current NM RN License (LPN considered)
  • 1+ years MDS experience or MDS Certification preferred
  • Understanding of MDS 3.0 and RAI regulations
  • Strong attention to detail and computer skills
  • Team player with nursing experience in a skilled setting
