Job Description

Job Position: Bilingual Care Manager (RN)

Pay Rate: $30-40/hour (based on experience)

Responsibilities:

• Manage care for high-risk patients, conducting screenings and risk assessments.
• Develop and implement individualized care plans in collaboration with patients, families, and providers.
• Facilitate communication among care teams and adapt practices to meet diverse patient needs.
• Utilize motivational interviewing techniques to engage patients and families.

Required Experience/Licensure:

• Bachelor’s degree in Nursing and RN license required.
• 2+ years of clinical nursing experience; bilingual in English and Spanish is mandatory.
• Preferred experience in managed care and case management.

Skills:

• Strong analytical and communication skills; proficient in computer applications.
• Ability to work independently and detail-oriented.
