Job Description

Job Summary:

This work-from-home position requires travel within the Anderson, SC County area. The On-site Care Coordinator, a Registered Nurse, provides care coordination and education to Medicaid members in the Medically Complex Children’s Waiver program. Responsibilities include conducting assessments, developing care plans, and coordinating services for families.

Qualifications:

• Minimum of three years of pediatric nursing experience.
• Bachelor’s degree in a health-related field preferred.
• Current RN licensure and BLS certification required.
• Certified Case Management (CCM) certification needed within four years.
