Job Description

This position involves overseeing the PACE Home Care program, ensuring the development and revision of related policies, and managing the home care department. Key responsibilities include:

• Coordinating home care services and conducting assessments to identify participant needs.
• Supervising Home Care RNs and collaborating with interdisciplinary teams to create and implement care plans.
• Conducting vendor audits and ensuring compliance with regulatory guidelines.
• Managing staff hiring, orientation, and performance evaluations.
• Maintaining accurate documentation and facilitating communication regarding participant care.

Qualifications include a nursing degree, RN license in Massachusetts, and experience in geriatric care and management.
