We are seeking a compassionate and skilled Registered Nurse (RN) for a full-time case manager position in homecare. Responsibilities:

• Conduct patient assessments and collaborate on care plans.
• Promote patient health through education and self-care techniques.
• Supervise LPNs and HHAs, ensuring timely documentation and communication.
• Participate in staff education and on-call services.

Qualifications:

• Minimum of one year of clinical RN experience.
• Current RN license and CPR certification.
• Valid driver’s license and reliable transportation.
